Tibolone Description

Tibolone (INN, USAN, BAN) (brand name Livial, Tibofem), also known as 7α-methylnoretynodrel,is a synthetic steroid drug with estrogenic, progestogenic, and weak androgenic actions which was introduced in 1988 and is used widely in Europe, Asia, Australasia, and, with the exception of the United States (where it is not available), the rest of the world. It is used mainly for treatment of endometriosis, as well as hormone replacement therapy in post-menopausal women.

Tibolone has similar or greater efficacy compared to older hormone replacement drugs, but shares a similar side effect profile. It has also been investigated as a possible treatment for female sexual dysfunction.

How to take Tibolone

Step One Before you start taking the tablets, read the manufacturer's printed information leaflet from inside your pack. The manufacturer's leaflet will give you more information about tibolone and a full list of the side-effects which you may experience from taking it.
Step Two Take one 2.5 mg tablet every day. You may take tibolone at whatever time of day you find easiest to remember, but try to take your doses at the same time of day, each day. You can take the tablets before or after meals. You may find it helps to swallow the tablet with a drink of water.
Step Three If you forget to take a dose, take it as soon as you remember unless you are more than 12 hours late. If you are more than 12 hours late, leave out the forgotten dose and take your next tablet at the usual time. Do not take two tablets together to make up for a missed dose.

Before Taking Tibolone

Some medicines are not suitable for people with certain conditions, and sometimes a medicine may only be used if extra care is taken. For these reasons, before you start taking tibolone it is important that your doctor or pharmacist knows:

  1. If you (or a close family member) have had breast cancer.
  2. If you have had a cancer that you have been told is hormone-dependent.
  3. If you have any unexplained vaginal bleeding.
  4. If you have liver disease, or if you have any problems with the way your kidneys work.
  5. If you know you have high levels of fat (triglycerides or cholesterol) in your blood.
  6. If you have ever had a blood clot in your leg or lungs, or if you have had thrombophlebitis (inflammation of a vein).
  7. If you have angina, or if you have had a heart attack or stroke.
  8. If there is a possibility you might be pregnant.
  9. If you have diabetes mellitus.
  10. If you have migraines, or migraine-like headaches.
  11. If you have epilepsy.
  12. If you have gallstones.
  13. If you have a rare inherited blood condition, called porphyria.
  14. If you are taking any other medicines. This includes any medicines you are taking which are available to buy without a prescription, such as herbal and complementary medicines.
  15. If you have ever had an allergic reaction to a medicine.

Tibolone/Livial METHODS

In this randomized study, we assigned 4538 women, who were between the ages of 60 and 85 years and had a bone mineral density T score of −2.5 or less at the hip or spine or a T score of −2.0 or less and radiologic evidence of a vertebral fracture, to receive once-daily tibolone (at a dose of 1.25 mg) or placebo. Annual spine radiographs were used to assess for vertebral fracture. Rates of cardiovascular events and breast cancer were adjudicated by expert panels.
